User input about forum/staff/game development

This form is for those who have issues with the community/game development/forum&irc staff and wish to express them in a reasonable manner. The 'Forum account' field is completely optional, if this form element is not filled, I will have no way of knowing who you are, since all I see is timestamp + the responses placed in the fields.

I love this. I've already made two submissions and am very happy with them. I hope this never goes down and gets added to with more subjects.

Yes, I'm aware some people have added names, but please from now on, only add your name in the field provided rather than the content.
Some of the questions may be paraphrased and answered in public, I'm not sure yet.
