It happens when someone-usually a griefer- puts a block on your head. Just suicide with a grenade and the problem is solved.

Ugh no it isnt griefers or anything, it happens randomly and 80% of the time you disconnect or 20% of the time it just fixes itself.

But griefers do do it just to annoy other players. it dissappears when you are killed or the block is destroyed. I have been griefed like this many times. I know why it happens.
